The Community Living Services Program is a state-wide program with the ultimate goal of assisting people in their goals of remaining independent. The CLSP program performs home modifications and purchases durable medical equipment so that people can continue to live independently without being forced into nursing homes against their will.

Individuals are encouraged to apply to the program at any time, however they should understand that due to high demand, there is a substantial waiting list.
