Contract with Us

We are constantly pursuing projects which require us to accept bids. Almost always, these are construction projects and durable medical goods, however occasionally they are for other projects.

Construction Contracts

In order to bid on open construction projects, a contractor must supply us with the following documentation:

  • A Copy of current WV Contractors License
  • Proof of current insurance
  • Maintain an up-to-date W-9 form in our office
  • Demonstrate a familiarity with ADA-Compliant accessible construction and/or renovation.

A more complete information sheet is available for download here. (PDF)

If you can supply these items and you work within our service area, please contact Clark today at 304-296-6091 or by email, This e-mail address is being protected from spambots. You need JavaScript enabled to view it to receive additional details.

Durable Medical Equipment

Normally, these purchases include items such as hearing aids, wheelchairs, canes, walkers, potty-chairs, shower benches, shower lifts, portable ramps, etc. In every case, you must be able to provide delivery to the end consumer of the product. Also, we generally prefer to pay with a credit card. If your company is already equipped to handle Medicare/Medicaid billing, you are almost certainly qualified to bid on items we purchase. Having an in-house ATP is not required but is ideal. In every case, we expect a high level of customer service and a willingness to warranty any item purchased.

If your company would like to bid on durable medical equipment purchases, please double check our service area and then download and return this form to Clark by email, fax, or mail.

Please note that in order to be eligible to bid on DME purchases you must return this form.
